Job Title: Physician Assistant – Geriatric-Focused Rounding Role

Compensation: $105,000 to $120,000

Position Summary:
We are seeking a Physician Assistant (PA-C) with a passion for geriatric care to join our growing team of rounding providers. If you want to specialize in aging adult health and build lasting relationships with your patients , this is a perfect fit.

Responsibilities:

• Conduct in-person visits with seniors in assisted living and memory care
  
• Support chronic disease management, prevention, and end-of-life care
  
• Coordinate with nursing staff, specialists, and families
  
• Maintain clear documentation and clinical notes in EMR
  

Qualifications:

• Licensed as a PA-C in Minnesota
  
• Background in geriatrics, palliative care, or internal medicine preferred
  
• Strong communicator with excellent bedside manner
  
• Must have valid driver’s license and reliable transportation
